New waiver forms needed for sailing trip

Our sailing outfitter Courageous Sailing just revised their Waiver forms effective 5/28 and has requested us to use the new form.

Attached is the revised Waiver. A form has to be signed by each individual sailing. If the individual is a minor (under 18) they still have to sign their own waiver and countersigned by their parent/guardian. They will not accept multiple family names on the Waiver.

Please have the updated form signed by each individual participating on the trip and bring to the meeting tomorrow night
